نامه سرگشاده جمعی از پزشکان به سازمان پزشکان بدون مرز

نامه ما درباره شخصی است که به دليل شرکت در مراسم اول ماه روز جهانی کارگر در شهر سقز از طرف جمهوری اسلامی ايران بازداشت شده و از آوريل ۲۰۰۷ تاکنون در زندان سنندج بسر می برد.
محمود صالحی کارگر خبازشهر سقز بوده و يکی از رهبران اتحاديه نانوايان اين شهر می باشد.
ما اين نامه را برای شما می نويسم زيرا که محمود صالحی از يک بيماری شديد و پيشرونده رنج می برد و ايشان ازمعالجه مورد نيازش محروم
می باشد. در اين رابطه به گزارش پزشکی پيوست مراجعه فرمائيد.
رژيم جمهوری اسلامی اقدامات پزشکان معالج را برای رسيدگی و درمان ايشان متوقف می نمايد.
عليرغم اينکه وضعيت سلامتی محمود صالحی فوق العاده بحرانی است، تاکنون او و خانواده اش کمک و پشتيبانی لازم را برای مراقبت و درمان دريافت نکرده اند.
ما از سازمان شما برای کوشش در راه کمک به اين خانواده بی پناه و اين بيمار در بند از هر راهی که برايتان مقدور است ياری می طلبيم.
اقدامات سازمان شما که بعنوان تشکيلات جهانی برای کمکهای انسانی به مردم محتاج شناخته شده است می تواند در بهبود وضع محمود صالحی موثر باشد.
ما امضا کنندگان زير که اين نامه را حضورتان تقديم می داريم بعنوان پزشک چندين سال در کردستان ايران کار کرده و سالها است که در کشور سوئد نيز بکار طبابت اشتغال داريم.
بر اساس اطلاعات کاملا موثق پزشکی که از طريق همکارانمان در ايران بدستمان رسيده است، ايشان نياز عاجل به اقدامات جدی درمانی دارند.
اين نامه همچنين به سازمان پزشکان بدون مرز واحد سوئد، سازمان جهانی حقوق بشر، سازمان جهانی حقوق بشر واحد سوئد، صليب سرخ جهانی و واحد سوئد اين سازمان ارسال می داريم و درخواست کمک می نماييم.

گزارش پزشکی
محمود صالحی از بيماری کليه پلی کيستيک که يک بيماری ارثی است رنج ميبرد. بيماری ايشان چندين سال قبل تشخيص داده شده و سال ۱۹۸۱ به همين دليل تحت عمل جراحی قرار گرفته است. محمود صالحی گرفتار درد مزمن شکم بدليل بزرگ شدن کيستهای درون کليه بوده، همچنين گرفتار خستگی مفرط ، ضعف شديد جسمانی، عفونتهای مکرر کليه و دستگاه ادراری و سنگ کليه ميباشد.
بيماری ايشان همچنان که شما ميدانيد بيماری پيشرونده ای است که نهايتا به نارسايی کليه منجر می شود. محمود صالحی همچنين دچارعوارض متعدد ديگری چون بالا بودن شديد فشار خون، بالا بودن چربی خون، وجود پروتئين و خون در ادرار در چند سال گذشته بوده است.
در خلال دوره زندان بدليل وضع بحرانی جسمی چندين بار به بيمارستان انتقال داده شده، منتها هر بار فقط زمان کوتاهی در بيمارستان مانده و قبل از آنکه اقدامات درمانی جدی در موردشان بعمل آيد، دوباره به زندان برگردانده شده است و مقامات حکومتی اجازه ادامه مراقبت و اقدامات درمانی را از وی دريغ داشته اند. آخرين بار در هفته گذشته به دليل وضعيت فوق العاده بحرانی جسمی دوباره به بيمارستان انتقال داده شد. همسر ايشان توانسته است بمدت کوتاهی محمود را ملاقات نمايد، در حالی که از دست و پا به تخت بسته شده بود( به عکس همراه نامه توجه فرمائيد)
در خلال ملاقات محمود گرفتار حالت تهوع و استفراغ بوده و از سردرد غير قابل تحملی عليرغم بکار بردن مسکنهای قوی رنج برده است.
همسر محمود صالحی همچنين موفق شد که با پزشک معالج محمود ملاقات نمايد. به گفته ايشان وضع سلامتی محمود صالحی فوق العاده بحرانی است و نامبرده توضيح داده که فشار خون ايشان ۱۰/۲۵ بوده، درحالی که عفونت کليه و دستگاه ادراری و سنگ کليه سلامتی وی را بيشتر به خطر انداخته اند. بر اساس گفته پزشک معالج محمود صالحی گرفتار نارسايی شديد کليه بوده و به دياليز منظم و به احتمال زياد پيوند کليه نياز دارد.

Regarding imprisonment of Mahmoud Salehi in Sanandaj
Our letter is about a person who was imprisoned in April ۲۰۰۷ due to his participation in the first of May celebration of the workers movement in Iran and more specific in the city of Saghz. Mahmoud Salehi works as a baker and is one of the local union leaders of the bakery employees in Sagaz in the Kurdish region of Iran. This is the third time he is arrested due to the same reasons. We are writing to your organisation because Mahmoud is suffering from a severe illness and he is not getting the proper care and medicine that he needs. The government is stopping the doctors from giving him the proper care and medicine (see page ۲ for medical history and condition).
The situation for Mahmoud is extremely critical. He and his family do not receive any help or support and they have no possibility to influence the care and medicine he needs. We appeal to your organisation to try and help Mahmoud and his family’s hopeless situation in any way you can. The recognization your organisation has worldwide as a humanitary aid for people in need have the influence to improve the situation of Mahmoud Salehi.
We who write this letter have been working as medicine doctors for several years in Iranian Kurdistan and since many years being living and working in Sweden. According to many reliable medical reports via our colleagues from Iran, we have been convinced that Mahmoud Salehi needs an urgent intensive care.
This letter will also be send to Medecins Sans Frontiers of Sweden as well as Amnesty International, Amnesty Sweden, Red Cross with the appeal of help.

Medical history/condition
Mahmoud is suffering from autosomal dominant polycystic kidney disease (ADPKD). This was diagnosed for many years ago and he was operated ۱۹۸۱. He developed chronic flank pain as the result of enlargement of cysts in the kidneys. He has had constitutional symptom as fatigue, weakness and repeatedly urinary tract infection and nephrolithiasis. His disease, as you know, is a progressive disease and he has developed renal insufficience. He also suffers from several complications due to the disease as hypertension, hyperlipidemia, proteinuria and haematoria since a few years ago.
During his imprisonment he has at several times been moved to the hospital due to his critical condition but only been kept there for a short while before moving him back to prison. The care he has received at the hospital is insufficient; the government does not allow the doctors to give him the proper care and medicine that he needs.
Recently he was moved to hospital again and now his condition is really critical. His wife briefly met him at the hospital while he was coughed to the bed both by arm and leg (se picture enclosed). During her visit he was feeling nausea, vomiting and had a severe headache despite strong analgetic and opoids. She had the opportunity to speak with his doctor and he said that his condition is very critical. The doctor explained that his blood pressure was ۲۵۰/۱۱۰, and that he was suffering of urinary tract infection and nephrolithiasis. The creatinine´s level is very high, and the kidneys are enlarged. He only urinates small amounts.
According to the doctor, Mahmoud has sever renal failure which requires regularly hemodialysis and then most probably kidney transplantation
